Featuring case studies that navigate the difficulty in working with extreme states with patients at all stages of life, the book offers a dialectical theory of intersubjectivity that conditions the structuralization process in the formation of personality and its impact on attachment capacities and psychological treatment. In outlining a typology of psychic structure based on early relational trauma, Mills argues that the formation of the self in relation to internalization, object relatedness, and interpersonal relationships lead to very different psychic organizations in the formation of the subject and personal identity.

Jon Mills, PsyD, PhD, ABPP, is a Canadian philosopher, psychoanalyst, and clinical psychologist. He is Honorary Professor, Department of Psychosocial & Psychoanalytic Studies, University of Essex, UK, on faculty and clinical associate in the Postgraduate Programs in Psychoanalysis & Psychotherapy, Gordon F. Derner School of Psychology, Adelphi University, USA, and on faculty and a Supervising Analyst at the New School for Existential Psychoanalysis, USA. Recipient of numerous awards for his scholarship including 5 Gradiva Awards, he is the author and/or editor of over 35 books in psychoanalysis, philosophy, psychology, and cultural studies.
